As always we can provide pre and post care for you and your journeys needs

When making a booking please make sure you have the following at hand.

Please allow as much notice as possible, we do get rather busy and it could cause disappointment.

	Date and time of travel
	We will require the patients full name, date of birth, brief medical history that would be needed for the journey*,
	Your name and contact details
	To whom are we invoicing to
	Where we are travelling from and to, include all address details including the postcode this is important
	Advise of any special needs such as oxygen, care to bed sores etc.
	Is the patient taking any medication?
	Advise us of any infections such as MRSA
	The type of transport Stretcher, wheelchair or seated
	Do you require a child seat or Child harness (10lbs to 40lbs)
	Has the patient any mobility aids ?
	Advise of how much luggage or personal items are to carried with the patient
	Advise of any unstable mental state of the patient
	Are any escorts travelling with the patient?

* if a Paramedic is required further medical details may be required.
